Then complete the information, and mail it to the address listed on the form. Please complete a separate form for each policy to be changed.
Automatic Payment Plans - Enroll to have your premium payments automatically deducted from your bank account or charged to your MasterCard or Visa account. No need to write monthly checks, no due dates to remember, and no records to keep - we'll do it for you! You may even be eligible for a discount when you sign up for this service. (Savings account deduction and credit card payments are not available in Canada at this time).

- Use the Automatic Payment Form for bank account deductions or the Credit Card Authorization Form to charge your credit card.
Change of Beneficiary - Make changes or corrections to primary and contingent beneficiary information with this form. If more than one person is insured under this policy, please call for a special form.
Policy Change Form - Use this form to record legal name changes, and to correct spelling, date of birth, and gender information on your policy.
