ELECTRONIC DELIVERY CONSENT AND ENROLLMENT FOR GERBER LIFE ESERVICE AND EBILLING
Gerber Life Insurance Company is pleased to invite you to enroll in Gerber Life eService and eBilling, available on our website www.gerberlife.com (the "Site"). With your enrollment in Gerber Life eService, you have the opportunity to make specific changes to your account at any time.
Account information and services available through eService, include:
Electronic Receipt and access to your policy and related documents
Paperless Billing Enrollment (eBilling)
Automatic Payment Enrollment - Credit Card or Pre-authorized Checking (PAC)
Ability to make single one-time payments
Ownership Change
Beneficiary Change
Change of billing and payment schedule
Updating contact information, including address, email address, telephone numbers
Applying for and repaying policy loans
PLEASE READ THE FOLLOWING TERMS AND CONDITIONS FOR USING THESE SERVICES.
These terms and conditions constitute your agreement regarding electronic delivery of various documents and services under Gerber Life eService and Gerber Life Paperless Billing (this "Agreement"):
The terms "you" and "your" refer to the primary owner of the policy. The terms "we", "us", "our" and "Gerber Life" refer to Gerber Life Insurance Company and its affiliates.
The term "documents" includes, but is not limited to your insurance policy and notices, premium notices, billings, statements, and other disclosure documents, annual and semi-annual reports, privacy notices and other related materials regarding your insurance policy.
By enrolling in Gerber Life eService:
You acknowledge receipt of this notice concerning the operation of Gerber Life eService, and that you will receive your documents electronically, and
You consent to receive and send information electronically regarding your policy and to effect changes to your policy electronically, and
You consent to electronic delivery of all documents (to the extent permitted by law) by notification at the electronic mail address provided by you that such documents are available on the Site for your retrieval, and
You agree that in the event that you purchase additional policies from Gerber Life, this Agreement will apply to those policies and their associated documents, which will be automatically enrolled in eService and eBilling, and
You understand that enrollment and participation is voluntary, and you may cancel at any time, and
You acknowledge that your action in eService requesting policy changes constitutes your agreement to those changes, and
You agree that, in the event of a policy modification or renewal, your consent to electronic delivery of various documents and services under Gerber Life eService and Gerber Life Paperless Billing will remain in effect, and
You acknowledge that you are electing to allow for documents to be sent to the electronic mail address provided by you and that you are aware that Gerber Life rightfully considers this election to be your consent that all documents may be sent electronically, including notice of nonrenewal and notice of cancellation (where allowed by law). Therefore, you should be diligent in updating the electronic mail address provided to Gerber Life in the event that the address should change.
By enrolling in Gerber Life Paperless Billing (eBilling):
You acknowledge receipt of this notice concerning the operation of Gerber Life eBilling, and
You agree to maintain your eService account to enable Gerber Life to process eBilling on your policy, and
You agree to provide and maintain a valid e-mail address within your eService account to facilitate our communication with you, and
You consent to receive information electronically regarding premium notices for any policies enrolled, and
You understand that enrollment and participation is voluntary and optional, and may be cancelled at any time.
Opt-in Enrollment: Gerber Life eService, including eBilling, are voluntary and optional services; you may discontinue your enrollment at any time or choose not to opt-in, at no cost, to the extent required by law. Although Gerber Life eService will be available immediately upon enrollment, please allow up to two (2) billing cycles for enrollment in Gerber Life eBilling to take effect. You may also continue to have your policy serviced or receive any records described in this Agreement by paper, at no cost, to the extent required by law, via United States mail or telephone while enrolled in Gerber Life eService, and you may receive paper copies of your premium notices if you cancel Gerber Life eBilling. Gerber Life reserves the right to change these services at any time.
Electronic payments: By submitting the necessary enrollment information through Gerber Life eService, you are indicating that you would like to have the opportunity to remit online payments for your policy using electronic fund transfers from your checking or savings account, or by debit or credit card. You will only be able to remit payments for Gerber Life products from a checking or savings account or debit or credit card account at a financial institution in the United States.
Computer storage and printing: While using Gerber Life eService, you will have the ability to print your policy, documents and billing information, as well as the Gerber Life receipt of the change requests successfully submitted.
Corrections: When you request that Gerber Life make changes to your policy through Gerber Life eService, you will be given the opportunity to review all changes and to make any corrections before final submission.
Undeliverable Electronic Notifications: If Gerber Life determines that an email notification sent to you is undeliverable, we may, in our sole discretion, choose to mail the applicable document to you using United States Postal mail or other mail services and/or treat this consent as revoked and cancel your enrollment in eService and/or eBilling, revert your preferences back to paper delivery for all documents and begin sending them via United States Postal mail or other mail services to the address of record for your policy or account. If this occurs and you wish to continue receiving documents electronically, you will need to update the e-mail address you have on file and re-enroll in eService and eBilling. An undeliverable electronic premium notice will not change the policy premium due date or your obligations to make timely premium payments for any policy.
Paper copies: You may request a paper copy, at no cost, to the extent required by law, of your Gerber Life policy, as well as any eService document or premium notice by contacting us at 1-800-704-2180 or writing us at Gerber Life Insurance Company, 445 State Street, Fremont, Michigan, 49412.
Update contact information: You may update your contact information in the following ways:
By making the change yourself directly within your eService account

Cancellation: You may cancel your enrollment in Gerber Life eService and/or eBilling at any time and at no cost, to the extent required by law; however, doing so will not change (1) your obligations under any policy including, but not limited to, making timely premium payments for any policy or (2) the legal effectiveness, validity, or enforceability of electronic records that were made available to you prior to the implementation of your cancellation. If you cancel your enrollment in Gerber Life eService or eBilling, Gerber Life will deliver all documents and/or premium notices to you using United States mail. You may subsequently receive documents electronically only by re-enrolling in Gerber Life eService and/or eBilling.

Note: Cancelling Gerber Life eService will take effect immediately upon request. Cancelling eBilling may result in unavoidable processing delays of your account changes and premium payment submissions. Please allow up to two (2) billing cycles for the cancellation of Gerber Life eBilling to take effect.
Security: Gerber Life follows reasonable and regulatory required precautions to safeguard the confidentiality of electronic communications and information relating to your policy and documents. However, we are not responsible for unauthorized access to Gerber Life systems, nor are we responsible for damages of any type caused by unauthorized access.
System requirements: To participate in Gerber Life eService and eBilling on the internet and in order to access and retain electronic records in connection with such services, you will need a computer with a web browser that supports at least 128-bit encryption. You must also have the ability to receive and read e-mail as well as Adobe Portable Document Format (PDF) attachments.
Amendments to Agreement: You agree to abide by the terms and conditions of this Agreement as it may be amended from time to time. Therefore, you should review the terms and conditions regularly, as they may change at any time in our sole discretion. Amended terms will be posted on the Site, and you will be provided notice and have the opportunity to revoke your consent or change your delivery preferences. Your continued use of the Site will constitute your acceptance of the then-current Agreement, as amended, together with all of its terms and conditions.
